US Counties Ranks By Health Outcomes And Determinants 2010-2022
The dataset contains US counties ranking data based on measures of health outcomes and health determinants. The measures used to establish counties ranks are related to length and quality of life for health outcomes and to health behavior, clinical care, socioeconomic and physical environment factors for health determinants. US counties are described along with their FIPS (Federal Information Processing Standard) code and the US state they belong.

Description
Using America’s Health Rankings, University of Wisconsin Population Health Institute is providing every year since 2010, for nearly every county in US health ranking data. The annual Rankings provide a revealing snapshot of how health is influenced by where we live, learn, work, and play. They provide a starting point for change in communities.
The counties health rankings are based on counties and county equivalents (ranked places). Any entity that has its own FIPS county code was included in the rankings. The data are only for counties and county equivalents within a state and the major goal of the rankings is to raise awareness about the many factors that influence health and that health varies from place to place. Counties in each of the 50 states are ranked according to summaries of a variety of health measures. Those having high ranks, e.g. 1 or 2, are considered to be the “healthiest.” Counties are ranked relative to the health of other counties in the same state. Most of the data used was public data available at no charge. Measures based on vital statistics data, sexually transmitted disease rates, and Behavioral Risk Factor Surveillance System (BRFSS) survey data were calculated for this project by staff at the National Center for Health Statistics and other units of the Centers for Disease Control and Prevention (CDC). The health care quality measures included were calculated by the authors of the Dartmouth Atlas of Healthcare, using Medicare claims data. In many ranked counties, some individual measures do not have a large enough sample size to report data for that measure. In these counties, the state average was assigned for any missing measure in order to calculate a rank for that category.
The measures were standardized within each state to the average of counties in that state and a Z-score was obtained for each county. Each Z-score is relative to the other counties in that state and shown in the metric of standard deviations. A positive Z-score indicates a value higher than the average of counties in that state; a negative Z-score indicates a value for that county lower than the average of counties in that state. For counties with a population of 20,000 or less, any z-score that is 3.0 was truncated to -3.0 or 3.0, respectively. For most of the measures, a higher Z-score score indicates poorer health. However, for some of measures (e.g., high school graduation) a higher score indicates better health or a more desirable value. For these measures was computed the Z-score as usual but multiplied by -1, so that higher scores indicate poorer health. The scores computed are weighted composites of the Z-scores for individual measures where the weights represent relative importance of the different measures chosen by authors. The weighted composite was computed by multiplying each Z-score by its weight and adding them up. After computation, the composite scores were sorted from lowest to highest within each state. The lowest score (best health) got a rank of 1 for that state and the highest score (worst health) got whatever rank corresponds to the number of units were ranked in that state. To de-emphasize the differences between individual county ranks, counties were at the same time grouped into quartiles according to their Health Outcomes and Health Determinants ranks separately. For each set of ranks there are four quartiles that divide up all the units within the state into the top 25%, the second from top 25%, the second from bottom 25%, and the bottom 25%. The top 25% are the healthiest counties with the best ranks, the bottom 25% are the least healthy counties with the worst ranks, and the other two quartiles are in between.
